A full-busted woman in her fifties rolling her shoulders back with relief
Testers rated their upper-back ache from 0 to 10 after every day of wear, for four weeks.

In the clinic, I could often guess a woman’s cup size before she took her coat off. The deep red grooves in her shoulders, the knot between her shoulder blades, the way her shoulders curled forward and her neck jutted out. She’d come in about her neck or her back. The cause was hanging off two thin straps.

Here’s the mechanics. A heavy bust can weigh several pounds, and in most bras that weight hangs from the straps. The straps pull your shoulders forward and down, and your upper back rounds to carry the load. The muscles between your shoulder blades work all day just to hold you upright. That’s the ache. Massages and stretches help for a day; the bra puts it straight back.

So we tested on the women who feel it most. We put 13 support bras, posture bras and braces on 29 women from DD to H, aged 42 to 69, all with regular upper-back or neck ache. Each tester wore each finalist for at least five full days and rated her ache from 0 to 10 after every day. In their own bras, the average was 6.6.

Most bras nudged that number down a point or two. One took it from 6.6 to 2.1, and our testers’ strap grooves faded in the first week.

How we tested for upper back pain

Each finalist was worn for at least five full days per tester and scored out of 10 on the four things that decide whether a full-bust bra hurts or helps.

Upper-back relief

Testers rated upper-back and neck ache from 0 to 10 after every day of wear. The bigger the drop from their own bras, the higher the score.

No strap grooves

We photographed testers’ shoulders after every wearing. Red marks, grooves and dents cost points, because they mean the straps are carrying the weight.

Support & lift

A heavy bust has to be held up and in. Lift was measured with a fitter’s tape, and bounce and spillage were logged.

The ShapeChic Posture Bra took our testers’ upper-back ache from 6.6 to 2.1 out of 10, a 68% drop. It was also the only bra that left every tester’s shoulders free of grooves.

It starts by taking the weight off your shoulders. A firm, wide underbust band sits flat around your ribs and carries the bust from underneath, the way every fitter says a bra should and almost none do. With the band doing the lifting, the straps stop dragging your shoulders down, and the dents they’d carved into our testers’ shoulders faded within the first week.

Then it fixes the posture the weight created. Two wide straps cross in an X over your upper back and draw your shoulders back and down, countering the forward drag of a heavy bust. Chests opened, necks lengthened, and the muscles between the shoulder blades finally got a rest. “It’s like someone took a backpack off me,” one 38H tester wrote.

It fastens at the front, with three hooks at centre chest and eight levels of adjustment, so you never wrench an aching shoulder reaching behind you. The wireless, seamless moulded cups hold a full bust without a wire biting your ribs, and the high sides and wide band smooth back and side bulge under fitted tops.

Sizes run S to 5XL, up to H cups, in Nude and Black. Right now it’s Buy 1, Get 1 Free, so two cost less than a single Wacoal. For a lot of our testers, that was cheaper than one more massage.

“I’ve had grooves in my shoulders since I was twenty. One week in this bra and they’ve faded. So has the knot between my shoulder blades.”

Sharon, 54Tester, 40G

Glamorise’s MagicLift Front-Close Posture Back was the runner-up for back relief, bringing testers’ ache down to 4.3. Its firm build holds a heavy bust securely, and it fastens at the front.

It still leaned on the straps, though, and about half our testers had marks on their shoulders after wearing it. It ran warm, felt bulky under clothes, and costs around $55 for one bra.

What we loved

  • Second-best back relief
  • Secure hold for a heavy bust
  • Front closure

Where it fell short

  • Left marks on about half our testers
  • Warm and bulky
  • Around $55 for one bra

Bottom line: A solid support bra, but your straps are still doing too much of the work.

The Exquisite Form Fully is the budget pick at around $38, with a front closure and a posture back that eased testers’ ache to 4.8 on average.

On our heavier busts, though, the straps took most of the weight and the grooves were back within a few days. Lift was moderate, and several testers said they bounced more than they liked.

What we loved

  • Front closure
  • Some back relief
  • Low price

Where it fell short

  • Straps still grooved heavier busts
  • Moderate lift
  • More bounce than testers liked

Bottom line: Better than an ordinary bra for back ache, but not built for a truly heavy bust.

The Wacoal Back Appeal gave strong lift and shape up front, and its wide back band smoothed beautifully under fitted tops.

For back pain it was a step sideways. Testers’ ache only fell to 5.6, because nothing pulled their shoulders back and the straps still carried a lot of the load. The underwire dug in for most testers over 50, the hooks are at the back, and it’s around $72 for one bra.

What we loved

  • Strong lift and shape
  • Excellent back smoothing
  • Polished look

Where it fell short

  • Little back relief: ache only fell to 5.6
  • Underwire dug in for most testers over 50
  • Around $72, with back hooks

A typical $25 posture brace pulls the shoulders back hard, and while it was on, testers felt their chests open. It brought their average ache down to 5.2.

But it does nothing about the weight. You still wear your usual bra underneath, so its straps keep grooving your shoulders, and the brace adds its own straps cutting under your arms. Most testers wore it for an hour or two at a time before pulling it off.

What we loved

  • Strong pull-back while worn
  • Cheap
  • Adjustable

Where it fell short

  • Doesn’t carry the bust’s weight
  • Cut under the arms
  • Not a bra: you still need one underneath

Bottom line: It treats the posture, not the weight that caused it.

How to choose a bra for upper back pain

If your bust is heavy and your upper back aches, these are the five things a physio would check first.

  1. The band should do most of the work

In a well-built bra, about 80% of the bust’s weight is carried by the band around your ribs, not the straps. A firm, wide band is the single biggest fix for back ache.

  2. Grooves are a warning sign

Red dents in your shoulders mean the straps are carrying the weight, and dragging your shoulders forward with it. Wider straps alone don’t fix that; a better band does.

  3. Pull the shoulders back, not just up

Straps that cross over the upper back counter the forward pull of a heavy bust and take the strain off the muscles between your shoulder blades.

  4. Skip the wire if it bites

An underwire that digs into your ribs makes you hunch away from it. Wireless moulded cups held up by a strong band support you without the bite.

  5. Fasten at the front

Twisting behind your back to hook a bra strains the exact muscles that already hurt. A front closure takes that strain away.

Questions readers ask

Can a bra really help upper back pain?
If the pain comes from a heavy bust hanging off your straps, the right bra makes a huge difference. In our test, the Posture Bra cut testers’ average upper-back ache from 6.6 to 2.1 out of 10.
How does it take the weight off my shoulders?
A firm, wide underbust band carries the bust from underneath, so the straps don’t have to. Then two wide straps cross in an X over the upper back to pull your shoulders back and down.
Will it get rid of my strap grooves?
Our testers’ grooves faded within the first week, because the band, not the straps, carries the weight. Not one tester had shoulder marks after wearing it.
Is it supportive enough for a really heavy bust?
Yes. It goes up to H cups, from S to 5XL, and our panel ran from DD to H. The wireless moulded cups hold a full bust without a wire digging in.
Is it easy to put on with a sore back?
It fastens at the front with three hooks at centre chest and eight levels of adjustment, so there’s no twisting behind you.
What if it doesn’t work for me?
Rate your ache for a week in your old bra, then a week in this one. If the number hasn’t dropped, send it back within 30 days for a full refund. Shipping is free and tracked.

Our verdict: the best bra for upper back pain

ShapeChic Posture Bra in black, front closure

Full-bust back pain comes from one thing: weight hanging off your straps and dragging your shoulders forward. The ShapeChic Posture Bra was the only bra that fixed both halves of that problem, carrying the weight on a firm band and pulling the shoulders back with its X-straps. Our testers’ ache dropped by 68%.
